A twenty-three-year-old graduate is posted alone to a rural hospital in Smolensk province with a feldsher, two midwives and a surgical atlas he reads by lamplight between cases. Linked accounts follow him through a tracheotomy on a suffocating child, an amputation, a breech delivery and a blizzard, each entered in terror and got through by pretending otherwise. Bulgakov held that post himself in 1916 and wrote the sequence a decade later, while making his living from a Moscow railwaymen's paper.
